Cash Only Patients
Cash only is a type of payment where the patient’s own resources pay for the care. It is contract between the patient and the physician. Individuals pay for care with personal funds, health saving accounts, and other funds. Cash only is the basis upon which the healthcare financing system began. Patients paid physicians a fee-for-service. In its purest sense, the Cash Only model includes only the physician and patient in the exchange of compensation for medical care provided. Over the years as healthcare financing arrangements have changed, entities paying a fee-for-service includes all payers-public and private. Recently, the healthcare industry has referred to physician practices that do not accept health insurance as cash-only practices or Cash only offices. Cash only vastly reduces the traditional overhead expenses by not having to bill and abide by contractual requirements of third party payers. There are already millions of Americans who cannot afford private health insurance and have Medicaid coverage. Others self-insure due to their financial resources being such that they can pay out-of-pocket for their medical expenses.

Jock Itch
Tinea cruris is the scientific name for a fungal infection in the groin region. An official alternative name is jock itch. As the common name for this condition implies, it causes itching or a burning sensation in the groin area, thigh skin folds or anus. It may involve the inner thighs and genital areas, as well as extending back to the perineum and perianal areas. Affected areas may appear red, tan and brown, with flaking, rippling, peeling, or cracking skin. The acute infection begins at an area in the groin fold measuring about a half-inch across, and usually presents on both sides. The area may enlarge, and other sores may develop in no particular pattern. The rash appears as raised red plaques (plate-like areas) and scaly patches with sharply defined borders that may blister and ooze. If the rash advances, it usually advances down the inner thigh. The advancing edge is redder and more raised than areas affected for longer. The advancing edge is usually scaly, and very easily distinguished or well demarcated. The skin within the border turns a reddish-brown and loses much of its scale. The border may exhibit tiny pimples or even pustules, with central areas that are reddish and dry with small scales. If infected with candidal organisms, the rash tends to be redder and wetter. Opportunistic infections (infections that are caused by a diminished immune system) are frequent. Fungus from other parts of the body (commonly tinea pedis or "athlete's foot") can contribute to jock itch. A warm, damp environment allowing the fungus to cultivate greatly contributes to the infection, especially with sweaty or tight clothing such as a jockstrap.
Family Physicians
Family physicians deliver a range of acute, chronic and preventive medical care services. In addition to diagnosing and treating illness, they also provide preventive care, including routine checkups, health-risk assessments, immunization and screening tests, and personalized counseling on maintaining a healthy lifestyle. Family physicians also manage chronic illness, often coordinating care provided by other sub-specialists. Family medicine and family physicians play a very important role in the healthcare system of a country. In the USA, for example, nearly one in four of all office visits are to family physicians. A general practitioner is a medical practitioner who treats acute and chronic illnesses and provides preventive care and health education for all ages and both sexes. They have particular skills in treating people with multiple health issues and comorbidities. Family medical practitioners must then complete three to four years of additional residency in family medicine. Three hundred hours of medical education within the prior six years are also required to be eligible to sit for the board certification exam; these hours are during residency training. Family physicians will see anyone with any problem and are experts in common problems. Many family physicians deliver babies in addition to taking care of patients of all ages. In order to become board certified, family physicians must complete a residency in family medicine, possess a full and unrestricted medical license, and take a written cognitive examination. The American Board of Family Medicine, as well as other specialty boards, is requiring additional participation in continuous learning and self-assessment to enhance clinical knowledge, expertise and skills. Family physicians may pursue fellowships in diverse topics including adolescent medicine, geriatric medicine, sports medicine, sleep medicine, and hospice and palliative medicine.
